How To Order Department Forms

1. Log onto PCIS

2. Select "Web Ref"

3. Select 'UCSD Healthcare Intranet" near the bottom of the list and click on "Home"

4. Under the second heading "Organization" Select "Forms Management" It is the 5th selection

5. In the blue box that states "what form are you looking for" enter the form number. This number is printed at the bottom of our forms. For example the "transcription paper" that we use for patient chart paper is number 151-350

6. Next select "search". You then have a PDF to the right that you may view to verify it is the correct paper or form you are looking for. If you choose to view it then simply click on the PDF; to return select the back button.

7. To order the form select "order forms here" on the bottom left.

8. You will be required to log on. Use your Weboutlook address and password. Next you will be able to order the amount of papers or reams you need. It will state Ea if it is per piece and 500/RM if it is a packet.

9. Enter your desired amount and click on the cart icon to add to cart.

10. To finalize your order, select Check out

Next time you place an order, you can skip step 5 and 6 as all of your previous forms selections will be listed to choose from.
